Optometrist Disability Insurance is offered by Core Protection Partners ("we," "us"). Core Protection Partners owns this website and processes the information you share on it. This policy explains what we collect, why, and the choices you have.

Information you give us. When you request a quote, you give us your first and last name, email address, phone number and state. If you email us, we receive what you write.

How you found us. The quote form also sends the page you started on and the site that sent you. It includes any campaign codes or ad click IDs from the link you used. It records the time, your IP address and your browser type, so we have a record of your request.

Analytics. If you accept analytics cookies, tools such as Google Analytics and Microsoft Clarity record how visitors use the site, such as pages viewed and clicks. They do not receive what you type into the quote form.

This website does not ask for health information. If you apply for coverage, the application and any health questions happen later, directly with your agent and the insurance carrier.

Children
